ADMISSIONS CRITERIA

Always First Home Care, LLC will accept all referrals from Texas Health and Human Services and most Managed Care Organizations (MCO’s) under the PHC Program. The agency will only provide the tasks allowable in the program as described in §47.41 of the Texas Administrative Code, Chapter 47 (relating to Allowable Tasks) and agree to on the service delivery plan

INDIVIDUAL SERVICE PLAN

The agency will receive a service delivery plan with the purchased tasks and/or authorized number of hours per work week and the day plan will vary based on insurance. This authorization is received from your insurance provider and will be reviewed with you by an agency field supervisor. Your service delivery plan is based upon identified problems, needs, services, timeframes, your environment and your personal wishes whenever possible as determined by your insurance case worker, NOT the agency.
